SOS is a volunteer-based non-profit organization meeting the needs and improving the lives of District 69 residents since 1968. SOS provides more than 20 local programs and services to all ages, including programs that meet basic needs and promote healthy living and social connections.

 

The Community Connector ensures the effective delivery and administration of the Social Prescribing Program, ensuring program goals and objectives are met and maintained by coordinating services that meet client needs while maintaining organizational standards and budgets. You will build strong community and healthcare partnerships, manage a caseload of seniors through holistic assessment and individualized wellness planning, and support safe transitions and healthy aging. You will also perform a range of related administrative and support tasks.

Qualifications & Skills: 

  • Completion of post‑secondary education in social or human services with at least two years of related experience, or an equivalent combination of education and experience.
  • Knowledge of seniors’ issues and healthy aging, with experience working with seniors.
  • Ability to build rapport with seniors and collaborate effectively with other staff and agencies.
  • Excellent written and verbal communication, and people skills. Highly organized, able to manage competing priorities.
  • Strong administrative and computer skills, including proficiency with Microsoft and databases.
  • Valid driver’s license.

 

Hours of Work:      35 hours per week

Compensation:      $24.50/hour (+ benefits listed above)
